Go straight to a hospital with a CT scanner. Bleeding inside the skull can build up over hours while the person still seems to be talking and walking normally.

⚠ Go to hospital immediately if any of these appear ⚠

These can appear hours after the injury, in someone who seemed completely fine at first.

Lower the threshold and go straight in — regardless of how minor the injury looked — if the person is on blood thinners, is elderly, has a bleeding disorder, or had taken alcohol.

What can happen after a head injury

The danger is rarely the visible wound. It is what develops inside the skull afterwards.

Concussion

A temporary disturbance of brain function — brief confusion, memory gap around the event, headache, dizziness or nausea. Scans are usually normal. Recovery is the rule, but it needs graded return to activity rather than pushing straight back.

Extradural haematoma

Rapid bleeding between skull and the covering of the brain, classically after a blow to the side of the head. The dangerous pattern is a lucid interval — the person seems fine for a period, then deteriorates fast. Highly treatable if reached in time, which is the whole reason for the observation advice.

Acute subdural haematoma

Bleeding beneath the brain covering, usually from a more forceful injury such as a road accident or fall from height. Often accompanied by underlying brain injury, and generally the more serious of the two acute bleeds.

Chronic subdural haematoma

Slow bleeding that accumulates over weeks after a fall the family may have forgotten. Common in older people and in those on blood thinners. Presents as confusion, drowsiness, unsteadiness or personality change — and is very treatable once recognised.

Skull fracture & contusion

A fracture matters mainly for what lies beneath it. Contusions are bruises of the brain that can swell over the following days, which is why some patients are admitted for observation despite looking stable on arrival.

Post-concussion syndrome

Headache, fatigue, poor concentration, irritability, light and noise sensitivity, and disturbed sleep persisting for weeks after a mild injury. The scan is normal and the symptoms are real. Recognising it prevents patients being told nothing is wrong.

Observing a minor head injury at home

For a minor injury with no warning signs, home observation is appropriate. Someone should stay with the person for the first 24 hours and be able to rouse them.

  • Sleep is fine — but check periodically that they rouse normally
  • Simple pain relief for headache, as advised
  • Return to work, screens and study gradually

Do notयह न करें

  • Leave the person alone overnight after the injury
  • Drink alcohol for at least 48 hours
  • Drive until symptom-free and cleared
  • Return to contact sport before full recovery

Common questions

My child hit their head but seems fine. Should I get a scan?

Usually not. Most minor head knocks in children do not need imaging, and CT scans carry a radiation dose that matters more in a child. What matters is observation. If the child is alert, behaving normally, playing, feeding and not vomiting repeatedly, watchful observation at home is appropriate. If any warning sign below appears, go to hospital immediately.

How long should someone be watched after a head injury?

The first 24 hours are the most important, and the first 48 remain a period of caution. Someone should stay with the person overnight and be able to rouse them. There is no need to keep them awake — sleep is fine — but they should be checked periodically and must be rousable. If they cannot be woken normally, that is an emergency.

A scan was normal but the headaches and poor concentration continue. Why?

This is post-concussion syndrome, and it is common after even a mild injury. Headache, dizziness, fatigue, poor concentration, irritability, sensitivity to light and noise, and disturbed sleep can persist for weeks. A normal CT does not exclude it — the scan shows structure, and this is a functional problem. Most people recover with graded return to activity, sleep correction and time.

When can I go back to sport or the gym?

Return should be graded, not sudden, and you should be free of symptoms at rest before starting. Returning to contact sport before recovery is complete is genuinely dangerous, because a second injury during that window can cause disproportionate harm. Come in for a return-to-play assessment rather than guessing.

Someone elderly fell weeks ago and is now confused. Is that related?

Very possibly, and this needs assessment soon. Slow bleeding around the brain can build up over weeks after what seemed a trivial fall, particularly in older people and in anyone on blood thinners. Presentation is often confusion, drowsiness, unsteadiness or personality change rather than dramatic symptoms. It is treatable, and it is missed when nobody connects it to the old fall.
