Most people with epilepsy become seizure-free on the right medication. Dr P K Jha (M.Ch, AIIMS · 30+ years) evaluates first seizures, adjusts treatment that is not working, and advises on driving, work and pregnancy — at Gaur City 1, Greater Noida.
Call for emergency help if the seizure lasts more than 5 minutes, if a second seizure follows without full recovery in between, if it is a first-ever seizure, if there is injury, difficulty breathing, or if the person is pregnant or diabetic.
Many seizures are missed for years because they do not match what people expect. Seizure type decides which medicine works — the wrong choice can make some types worse. कई दौरे वर्षों तक पहचाने नहीं जाते क्योंकि वे अपेक्षा के अनुरूप नहीं दिखते। दौरे का प्रकार तय करता है कि कौन सी दवा काम करेगी।
The familiar type — loss of consciousness, stiffening, then rhythmic jerking, often with tongue biting or loss of bladder control, followed by deep drowsiness and confusion for minutes to hours. Usually witnessed rather than remembered.
Consciousness is preserved. A rising sensation in the stomach, an odd smell or taste, sudden intense familiarity or fear, or jerking limited to one hand or side of the face. Brief and often dismissed as anxiety or gastric trouble for years.
Staring, unresponsiveness, and repeated automatic movements — lip smacking, fumbling with clothes, plucking at objects — lasting a minute or two, with no memory of it afterwards. Often mistaken for absent-mindedness or confusion.
Brief blank spells of a few seconds, many times a day, mostly in children. Frequently reported first by teachers as inattention or daydreaming rather than by parents. Falling school performance is sometimes the only clue.
Triggered by a reversible cause — very low blood sugar or sodium, high fever, alcohol withdrawal, severe sleep deprivation, or certain drugs. These are treated by correcting the cause, and do not always mean lifelong epilepsy medication.
Fainting, cardiac rhythm problems, and non-epileptic attacks can all look like seizures. Getting this right matters — it prevents years of unnecessary medication, and in the case of cardiac causes it can be life-saving.
Missed doses are the commonest reason treatment appears to fail. Around two-thirds of patients become seizure-free on the first or second appropriate drug, taken consistently. Never stop suddenly — abrupt withdrawal can provoke prolonged seizures more dangerous than the original ones.
Sleep deprivation is the trigger we see most, particularly in students and shift workers. Alcohol, missed meals, high fever and in some patients flashing lights also matter. Keeping a simple seizure diary reveals patterns nobody predicted.
Showers rather than baths, avoiding swimming alone, care around cooking fires and machinery, and telling at least one colleague what to do. Most people with controlled epilepsy work normally — this is about sensible precautions, not restriction.
Epilepsy is a treatable medical condition of the brain's electrical activity. It is not a mental illness, not contagious, and not a reason to hide a diagnosis, leave school, or refuse a marriage proposal. Families often need this conversation as much as patients do.
Not necessarily. Epilepsy means a lasting tendency to seizures, and a single seizure does not always mean that. A first seizure needs proper evaluation — history, examination, EEG, imaging and blood tests — to work out whether it was provoked by something reversible, and what the risk of recurrence is. That risk determines whether medication is advised.
Stay with them. Move sharp objects away, cushion the head, and turn them onto their side once the jerking stops. Time it. Do not put anything in the mouth, do not hold them down, and do not try to give water or tablets. Call for emergency help if the seizure lasts beyond 5 minutes, if another follows without recovery in between, if it is a first seizure, or if there is injury or difficulty breathing.
Not immediately after a seizure, and rules depend on seizure type, cause and how long you have been seizure-free. This is a safety matter for you and others, and it needs an individual answer at consultation rather than a general rule from a website. Driving before it is safe also affects insurance.
Pregnancy needs planning, not panic. Uncontrolled seizures carry their own risk to mother and baby, so stopping medication on your own is dangerous. Some drugs are preferred and some are avoided, doses may need adjusting, and folic acid is started before conception. Discuss it before becoming pregnant if at all possible.
Not always. Many patients who are seizure-free for a sustained period on stable treatment can be considered for gradual withdrawal, depending on seizure type, EEG findings and cause. This is a planned, supervised taper — never a sudden stop, which can trigger prolonged seizures.
